Little Dreams DR, Inc. is a Bronx nonprofit. Our outreach team connects Medicaid members to the food, housing and transportation support they qualify for through New York's Social Care Network program. What's left over goes to toys, clothing, food and everyday essentials for children and families in the Dominican Republic.

We focus on one program: New York's Social Care Network (SCN). We help New Yorkers on Medicaid get the food, housing and transportation support they qualify for.
Screen
With each person's consent, we screen for unmet needs like food, housing and transportation.
Connect
We connect eligible members to the Social Care Network and follow up to make sure they get help.
How it works
Service here, giving there
We're a nonprofit, so no one owns the surplus. Every dollar left after program costs goes back into our charitable mission.
