Connecting New Yorkers to the Social Care Network program
All of our work in New York is for one program: the Social Care Network (SCN). The surplus from that work funds our giving in the Dominican Republic.
About the program
New York's Social Care Network program
Social Care Networks are regional networks, funded through New York Medicaid, that connect members to services for health-related social needs. Services come at no cost to eligible members.
Needs the program can help with
- Food: medically tailored or home-delivered meals, groceries and nutrition counseling
- Housing: help finding or keeping housing, and fixing unhealthy home conditions
- Transportation: rides to social care services and appointments
- Care management: a navigator who helps members get the services they need
Every Medicaid member with an unmet need can get navigation. Some services are limited to specific groups, such as pregnant members or people with certain health conditions.
What we do today
Outreach, screening and connection
Many people qualify for help they've never heard of. Our bilingual outreach team closes that gap in the Bronx and across New York City.
How we help
- Community events, clinics and neighborhood outreach
- Explaining the program clearly, in English and Spanish
- Health-related social needs screenings, with each person's consent
- Connecting people to the Social Care Network and following up
Where the surplus goes
Little Dreams for the DR
The surplus from our work in New York goes to children and families in the Dominican Republic, delivered in person and through local partners.

Looking ahead
Delivering services directly
As we grow, we plan to deliver social care services ourselves, starting with nutrition support for the families we connect to the program.
